Why Must I Stop Wearing Contact Lenses Before My Lasik Evaluation?

Many prospective Lasik surgery patients wear contact lenses. When they've come to the point of considering Lasik surgery, they may have tired of the inconvenience of contacts or maybe they may have problems tolerating contacts for long periods. With all patients that undergo evaluations for Lasik, I ask them to discontinue their contacts for at least 2 weeks prior to the exam. Although 2 weeks may seem like a certain long time to be without contacts, it is extremely important to the output of the Lasik, so we insist on it.

An optical system like glasses is much closer to perfect compared to an eye because it can be ground and shaped in a laboratory. The eye, on the other hand, is made of human tissue and it is naturally imperfect, so we expect a certain amount of lumps, bumps, and warping to be present. These human imperfections of the eye are as unique to the individual as their fingerprints. Modern Lasik technology is capable of registering these little imperfections and accounting for them in the laser treatment. Because of the sensitivity of this equipment, it is particularly important that the corneal surface is allowed to reach its most relaxed, natural optical state. Modern lasers typically incorporates a wave scan system which allows the computer to measure optical refractions and aberrations in an ultra-sensitive manner. In simpleest terms, a wave scan system sends light into your eye and is very accurately able to measure the light that reflect back to it. By mathematically analyzing this returning light, the computer can derive an incredible amount of optical information about your eye. This optical information can in turn be used by the laser during a Lasik treatment to render very accurate treatments. With these systems, optical aberrations that may be unique to your eye can be analyzed and treated in ways that older laser technology could not.

Contact lenses sit directly on the outer corneal surface of the eye. They are a reliably firm “plastic” material and essentially a foreign body to the eye itself. So although they improve your vision while you wear them, they are directly rubbing on the outer eye surface and have been shown to create significant surface changes when they are worn. These surfaces changes are generally not permanent so when the contact is discontinued the eye returns to it native state. The outer corneal surface is composed of a very rapidly growing and renew cell layer called the corneal epithelium. This cell layer quickly regroups and rejuvenates so over time the surface returns to its normal physical and optical condition. In years past, we allowed patients to be out of their contacts for just 3 days prior to their Lasik valuations and we felt this time was adequate for the measurements that we required. However, when the more advanced technologies were developed, particularly the highly sensitive wave scan analyzers, it became important that even very subtle corneal surface changes be allowed to resolve over a greater length of time. Currently a minimum of 2 weeks is considered standard for patients that wear soft contact lenses. Longer periods of over a month or more may be needed for patients who wear hard or gas permeable lenses.

Soft lenses probably only alter the outer corneal epithelium by direct contact or possibly by affecting the amount of oxygen available to the cells. These effects can definately create measurable warping that can be detected on corneal mapping (topography) and could alter the wave scan measurements as well. In a small number of cases, changes in the epithelium could create topographical changes in the cornea that mimic pathological problems in the eye such as keratoconus, a potentially progressive weakening and warping of the cornea. Only by stopping contact lens wear and observing the measurements after an adequate recovery period can we be sure if the cornea is healthy and stable.

Hard or gas permeable (GP) contact lenses can alter the epithelium just. However, these lenses are known to also flatten or compress the entire body (stroma) of the cornea in a way that may change the exact amount of nearsightedness of the patient. In this case, wearing the hard or GP contact can actually change the apparent amount of nearsightedness of the patient to a falsely lower level. If the patient discontinues the contact for several weeks, the cornea will relax and resume its natural curvature and the patient's full nearsightedness will disclose itself. Because of the falsely lower nearsightedness readings for hard and GP contact wearers, proceeding with Lasik without an extended period out of the contact can lead to a higher chance of under correction and consequent need for additional treatment at a later date.

In summary, contact lens patients considering Lasik should discontinue wearing their contacts for at least 2 weeks (longer for hard and GP contacts) prior to their preoperative exam and surgery. This precaution will allow for the most accurate measurement of the patient's prescription as well as higher order optical aberrations. Getting the most stable measurements which are free of contact related distortions allows for the most accurate Lasik treatments, the best potential visual consequences, and the lowest chance of requiring a second Lasik treatment.

Glaucoma Symptoms and Its Possible Treatment

There are four (4) important factors in the diagnosis of glaucoma: intraocular pressure, the condition of the optic nerve, the patient's visual field and the angle where the iris meets the cornea. To detect these factors, the ophthalmologist will do the following diagnostic tests:

· Tonometry – used to measure intraocular pressure. Drops are put in the eyes to numb the eye and then the doctor measures the eye pressure, using an instrument called a tonometer. This instrument measures the inner pressure of the eye by determining how much pressure is necessary to cause a slight indentation on the outer part of the eye.

· Ophthalmoscopy – used to examine the inside of the eye, the optic nerve and the patient's field of vision. Drops are put in the eyes to dilate the pupil and then the doctor uses an instrument called an ophthalmoscope that lights up and magnifies the inside of the eye.

If the intraocular presses are not within normal range or if the optic nerve looks unusual, the doctor may do a perimetry or a gonioscopy:

· Perimetry – a special test that produces a map of the complete field of vision.

· Gonioscopy – a test to check wherever the angle where the iris meets the cornea is open or closed.

Treatment of Glaucoma

There is no cure for glaucoma, but it can be controlled. Ophthalmologists use medication and surgery to prevent further vision loss. These treatment methods are used to bring the intraocular pressure under control.

Medication

Nearly all glaucoma can be treated successfully with topical medications (applied directly to the eyes, such as eye drops or eye ointments) or oral medications (taken by mouth). There are six remedies available to treat glaucoma:

1. Miotics are eye drops that help open the drain canals and increase the flow of aqueous humor out of the eye. This type of medication includes pilocarpine (Isopto Carpine, Pilocar, Pilopine), carbachol and echothiophate.

2. Epinephrine compounds are eye drops that lower the intraocular pressure by increasing the rate of aqueous humor flow out of the eye. This type of medication includes dipivefrin (Propine) and Epifrin.

3. Beta-blockers (available as eye drops or pills) help decrease the rate at which the aqueous humor flows into the eye. This type of medication includes topical timolol (Timoptic), levobunolol (Betagan), betaxolol (Betaoptic-S), Iopidine and Ocupress. Additionally, this type of medication can also be taken orally. Oral medications include propranolol, timolol, atenolol and nadolol.

4. Carbonic anhydrase inhibitors (available as pills or eye drops) help reduce aqueous humor flow into the eye. This type of medication includes oral acetazolamide (Diamox), methazolamide (Neptazane) and chlorpropamide (Daranide), as well as topical dorzolamide (Trusopt).

5. Alpha adrenergic agonists are topical medications used to reduce the aqueous humor production and increase aqueous humor outplow. This type of medication includes apraclonidine (Iopidine) and brimonidine (Alphagan).

6. Prostaglandin analogues are topical medications that help lower intraocular pressure by increasing uveoscleral outflow (outflow through the soft tissues of the front of the eye [iris and ciliary body]). This type of medication includes latanoprost (Xalatan).

Three medications approved in 2001 are prostaglandin-related drugs (Travatan, Lumigan) and a selective alpha2 agonist called Alphagan. A study by the maker of Travatan indicated the possibility that it may work best for African-Americans with glaucoma. The drug Copaxone, normally used to treat multiple sclerosis, is being researched as a possible vaccine to protect the optic nerve from the effects of glutamate in people with glaucoma.

How Were Eyeglasses Invented?

Wikipedia defines eye glasses as: “… frames bearing lenses worn in front of the eyes, normally for vision correction or eye protection.” Simple description for a simple vision aid that is used the world over by people in all walks of life. Glasses are accepted as being part of life, some people will need them and others will not and glasses. Vision care has become a major industry and glasses are a fashionable accessory, both for those that require them in order to see and those that do not. Vision care continues to evolve – a laser can now reshape your cornea and enable you with perfect vision – but glasses are not likely to ever completely disappear. Indeed, the level of sophistication has been aided greatly by both science and technology and vision care and will continue to be a part of life for the foreseeable future. But where did glasses come from? History shows that the discovery and evolution of glasses to where they are currently has not always been a clear path.

Remarkably, the earliest historical records or references to magnification date back all the way to the 5th Century as ancient Egyptian Hieroglyphs depict “simple glass meniscal lenses”. The man who tutored Emperor Nero in ancient Rome discovered that small letters and type were easier to see when viewed through glass filled with water. Other precursors such as these two examples did appear throughout history but nothing significantly happened until the year 1286 when eyeglasses were referred in a sermon. The man giving the sermon, Dominican firar Giordano da Pisa, referred to the art of “making eyeglasses, which make for good vision.” His colleague, Friar Alessandro Della Spina of Pisa, began making glasses for anyone who required them and by 1301 there were regulations in place governing the making and sale of glasses.

By 1462 concave lenses were being employed to correct farsightedness, but more advances were on the way. American scientist Benjamin Franklin adopted bifocal lenses in 1784 so as to avoid the need to carry two kinds of glasses with him everywhere since he suffered from both nearsightedness and farsightedness, although claims that he actually invented bifocals is unsubstantiated. By 1825, the very first lenses capable of correcting astigmatism were produced by a British Astronomer, George Airy.

The frames which contained the lenses were also undergoing a constant evolution. The earliest frames were actually held in front of the eyes with a handle and did not actually sit on the bridge of the nose or ears. Occasionally, this gave way to a frame that had no arms and instead was held in place by pinching them onto the bridge of the nose. Frames with arms that passed over the temples were actually created in some manner as early as 1725, but the style was not adopted initially and glasses frames with handles were the norm, as were some other short lived ideas, such as frames that would be tied around ones head with a ribbon, which was then secured in place by wearing a hat (!). According to Wikipedia, during the early 20th century, Moritz von Rohr developed the Zeiss Punktal spherical point-focus lenses that dominated the eyeglass lens field for many years.

In the wake of these developments, the science of optometry was advanced considerably. Much of the leap forward in the field of optometry came due to Von Rohr, he made thinner lenses possible and that in turn led to a marked decrease in the distortion of the eye others see when looking at the eyes of a person wearing glasses. Sometime after the Von Rohr advances, scientists began searching and experimenting for ways not only to improve lenses and correct vision, but to enhance aesthetics as well. This continued refining of aesthetics eventually led to the creation of the contact lens, which in turn led to the advent of laser eye surgical procedures for permanent vision correction. Both of these advances will not be covered here as they both warrant their own separate essays.

Despite the increasing popularity of contact lenses and laser corrective eye surgery within the last decade or so, glasses remain very common and well established. The technology has improved remarkably, and it continues to evolve. For example, it is now possible to buy frames made of special metal alloys that can be returned to their correct shape after being stressed or bent – such accidental damage to frames at one time mean out right replacement. Most frames have spring-loaded hinges, another advancement that prolongs the life of a set of frames and improvements the ability of a standard pair of glasses to withstand the stresses of daily wear and the incidental accident such as dropping or sitting on your glasses.
